According to the Indiana Department of Natural Resources (DNR), conservation officers were called to the area near Dandy Trail around 12:00 p.m. after reports that a man had gone underwater and failed to resurface.

Witnesses told authorities that Holmes had entered the water in an effort to retrieve a fishing boat that had drifted away. What began as a simple act of assistance quickly turned tragic, as he was seen struggling to stay afloat before disappearing beneath the surface.
Emergency responders, including the Indianapolis Fire Department Dive Team, launched a search operation. After approximately 30 minutes, divers located Holmes underwater and brought him to the surface.
He was rushed to IU Health Methodist Hospital in critical condition, where medical teams worked to save his life. Despite their efforts, Holmes later succumbed to his injuries.
